Become a Patient
To make an appointment for yourself, a family member or a friend, please complete and submit our Become a Patient Request Form or call City of Hope at 800-826-HOPE (4673).

Is My Insurance Accepted?
View a list of insurance companies with which City of Hope currently contracts. Even if your plan is included, check with your insurance company to make sure you are covered. Your insurance company/medical group will tell you if you need any authorizations.

Preparing for Clinical Appointments
We place a strong emphasis on well-being and convenience. From extended open hours to psychological, physical and spiritual services, our clinics provide a wide range of support for patients and families.

Treatments and Services
From prevention, to screening, to treatment, to rehabilitation, City of Hope cares for patients with expertise in cancer, diabetes and other diseases and an array of world-class technologies and services.

Clinical Trials
At any given time, up to 40 percent of our eligible medical patients are involved in active clinical studies conducted by our multidisciplinary teams of investigators and physicians.

Medical Facilities
Light-filled, comforting, state-of-the-art medical facilities (including Helford Clinical Research Hospital, Michael Amini Transfusion Medicine Center (Blood Donor Center), Village Hospice, Geri and Richard Brawerman Center for Ambulatory Care and Rita Cooper Finkle and J. William Finkel Women's Health Center) surrounded by lush gardens provide healing environments designed to treat both body and spirit. City of Hope's Community Practices also bring care closer to many patients' homes, making thier cancer diagnosis, therapy and follow-up more convienient.

After checking that City of Hope treats your cancer or other diagnosis, you can contact New Patient Services by calling 800-826-HOPE (4673) or by completing the online request form to begin the process.
City of Hope offers in-person medical evaluations only.
If you are a member of an HMO, you will need to obtain written authorization from your primary care provider or medical group in order to be seen at City of Hope.
You can access Clinical Trials Online to find out about clinical trials that are being conducted at City of Hope right away; but you must first be seen by a doctor to discuss whether you are eligible to participate in a specific trial. Once the patient has been seen by a City of Hope physician, he or she will be provided with information about treatment and can inquire about clinical trials.
City of Hope does not offer routine pap smears. However, a referral can be made to our gynecology department if you are: already a patient, at high risk, on Tamoxifen with irregular bleeding, or having abnormal side effects from other hormonal therapy.
