If you or a loved one are interested in becoming a patient at City of Hope, the following pages explain how you can be seen for treatment.
First Step: Contact New Patient Services
- By Phone: 866-434-HOPE (4673)
- Online: Complete and submit a “Becoming a Patient” Request Form
- Please note, we regret that we are unable to provide any information about treatment options until a City of Hope physician has seen the patient. Also, please be informed that if you are seeking information for someone other than yourself, the patient must be aware that you are contacting City of Hope on their behalf.
What Happens Next
After you contact us, a New Patient Specialist will then contact the patient to ask a few questions and verify insurance or Medicare/Medi-Cal eligibility. The patient will be asked about their diagnosis; however, no other clinical information will be needed at this time. Please do not send any medical records prior to receiving a specific request from a City of Hope New Patient Specialist.

Scheduling an Appointment
Once your eligibility has been verified, a nurse will contact you to obtain additional medical details and to schedule an appointment. If you are a member of a managed care plan (an HMO or Health Maintenance Organization), prior authorization may be required before you can receive treatment. City of Hope has contracts with most insurance companies (see list). If insurance authorization is not obtained, personal payment arrangements may be made.
Again, at this point the nurse cannot render any medical opinions or advice, answer specific questions about treatment options, nor determine eligibility for clinical trials. Such questions can only be addressed after the patient has undergone an examination by one of our physicians.
Hours
The New Patient Services office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. Pacific Standard Time.
