REGISTRATION IS NOW CLOSED.  YOU MAY REGISTER ON-SITE AT THE CONFERENCE IF YOU WISH TO ATTEND.

A confirmation will be sent after your registration form is received. Registration fee includes registration luncheon, daily breakfast, conference session break refreshments, Meet the Faculty welcome buffet and beverage reception for conference attendee only.

$450 for physicians
$350 for non-physicians
$250 for residents/fellows/City of Hope staff
$900 for Industry***
 
The following is the fee schedule for guests.
$125 for spouse/guest
$50 for children 2 to 12 years of age **

**Fee includes welcome reception.and daily breakfast

***Industry is defined by City of Hope as any person employed by a for-profit orgnization, including biotech, financial, and pharmaceutical.  Industry registrations cannot be processed online.  To register attendance at a meeting, please call the office of Continuing Medical Education at 626-256-4673 x65622.

Cancellation Policy
Refund requests must be submitted in writing and postmarked no later than August 12, 2011.  Refund requests postmarked on or before August 12, 2011, will receive a registration refund LESS a $50 processing fee.  Requests postmarked after August 12, 2011, will not be honored. All refunds will be processed 30 days after the meeting concludes. In the event of unforeseeable circumstances that lead to the cancellation by the conferences organizers, all registration fees will be fully refunded.
