CME Planning Timeline:
              Guide to Planning CME Certified Activity

Application for a CME Certified Activity :
              For a single day (1hr +), a multi-day (3 day symposium), or even enduring material
              (journal, web program etc) CME certified activity.

Application for a Regularly Scheduled Series (RSS) : (tumor board, journal club, etc)
              To renew or request a new RSS. 

Budget Template :
              Submit with your Application for a CME Certified Activity only.

CME Certified Activity- Letter of Agreement :
              Partnership agreement for any department/division and CME Dept working together to                    create a CME Certified Activity

Disclosure of Commercial Support/ Conflict of Interest Resolution Form (2012) :
              All Course directors, moderators, Faculty and Planners must complete this form
              annually and must be on record with the CME Dept.
