Overview

City of Hope Department of Nursing Research & Education provides an annual program to educate competitively chosen multi-disciplinary teams from cancer settings across the nation in cancer survivorship. Using a goal directed method of teaching, each team will develop three (3) goals relating to implementation of survivorship activities. These goals will be followed at 6, 12, and 18 months post course.

This three-day course provides survivorship education based on the four (4) domains of quality of life model:

  • Physical well-being and symptoms management
  • Psychological well-being
  • Social  well-being
  • Spiritual well-being

Expert national faculty involved with the Institute of Medicine report, as well as clinicians who provide survivorship care in their settings, will present the information.

A past participants' panel discussion outlines achievements made since their initial involvement in the program.  Interactive networking is encouraged and faculty provide guidance in goal development.   

Expected Results
At the end of the conference, the attendees will be able to work on completing their established goals to begin to build the framework for changes, to improve quality of survivorship care in their cancer settings.

This level of goal driven change will be followed at 6, 12 and 18 months for achievement.  Continuous access to supportive faculty will be available for resources and support.  Administration support begins with the application process and their continued support is evaluated at 6, 12 and 18 months.

Acknowledgement
This is a National Cancer Institute funded study (Grant R 25- CA107109)  which provides for the course registration and up to $250.00 reimbursement for travel and hotel. As a research study, each team must be committed to providing the brief follow up tools and interviews necessary at 6, 12 and 18 months.

Objectives
At the conclusion of the course the participants should be able to:

  • Discuss the issues and state-of-the-science in cancer survivorship.
  • Identify individual goals for improved care for cancer survivors in participants' cancer centers.
  • Join a network of course participants to share the experience of dissemination of the survivorship curriculum to the staff of participating cancer centers.
Who Should Apply
Two member teams from cancer centers may apply: 

  • One team member must be from tier 1 (nurse, physician or administrator). 
  • The second team member may be anyone in tier 1 or tier 2 (social workers, pharmacists, psychologists, rehabilitation specialists and clergy). 
Teams are responsible for establishing survivorship goals and carrying out those goals. Support of the institution is based on letters of support from administrators.
