Deadline
The application deadline for the 2009 Survivorship Education for Quality Cancer Care Conference is April 10, 2009.

Requirements

  • A two-person team made up of either two from tier I or one from tier I and one from tier II.    Tier I = Administrator, physician, registered nurse
    Tier II = Psychologist, social worker, pharmacist, rehabilitation professional, clergy
  • Application Form - Click here to download (MS Word required) 
  • Biographical Information. (See application form).
  • A Statement of Interest in attending the course.
  • Institutional Goals jointly created by the two-person applicant team.
  • Two letters of commitment and support from Director of Nursing/Patient Care Services, Chief of Staff/Medical Director, or Institution Administrative Official.
  • Institutional Survey.
  • This NCI sponsored program provides for program attendance and expense reimbursement for up to $250.00 per participant. This program requires team participation on institutional assessments and phone follow up at 6, 12 and 18 months post course.

Completed applications may be submitted by regular mail or fax. Incomplete applications will not be considered.
