Medical Benefits
Aetna PPO or HMO Plan or Kaiser HMO options; Prescription drug program; Option for dependent care coverage (shared by Trainee).
Dental Benefits
Choice of Aetna Dental PPO, Aetna DMO; Preventive, basic, and major dental service levels.
Vision Benefits
Optional vision plan is available if Trainee elects this coverage (this benefit is voluntary).
Disability Insurance
Short Term Provides 60% of salary for illness up to 90 days of absence starting on the 8th day of illness. Long Term is available and provides 60% of salary for illness after 90 days of absence if Trainee elects this coverage (this benefit is voluntary).
Basic Life and Accidental Death and Dismemberment Insurance
$50,000 of group term life insurance provided by the COHMG is provided. There is a cost of coverage in excess of $50,000 if Trainee elects to increase this coverage.
Retirement Plan
Optional 403(b) retirement plan is available if Trainee elects to contribute (this benefit is voluntary).  The 403(b) Tax Deferred Annuity Plan allows you to set aside money from your pay to save for retirement.  Your deferrals are taken on a pre-tax basis, so you do not pay taxes on the money or any earnings until you take a distribution from the plan.  This lowers your current taxes.
The Plan is administered by TIAA-CREF.  There are a variety of investment options from which to choose to build your own portfolio, or you can decide to invest in a Lifecycle Fund appropriate for your expected retirement year.  You have online access to your account 24/7 through the TIAA-CREF/City of Hope website ( or you can speak with a representative during business hours (800-842-2252).  You may also arrange to meet with a TIAA-CREF counselor in person at no cost to you to discuss your retirement needs.
You may defer up to the government limits each year (for 2013 $17,500 under age 50 and $23,000 age 50 and over).  You can start, stop or change your contribution amounts at any pay period by submitting a new Salary Reduction form.  Loans and hardship withdrawals are available in this plan. To enroll, first go to the TIAA-CREF/COH website and open an account.  Then print off a Salary Reduction form, sign it, and return it to Benefits and Employee Services.
Supplemental Term Life Insurance
Additional term life insurance may be purchased by Trainee and dependents (this benefit is voluntary).
Employee Assistance Program
Five (5) covered visits per year.
