Complete Check-in procedures for Newly Arrived Visa Holders

New J-1 Check-in procedures: Once you arrive at City of Hope, please send the following documents to Immigration Services at [email protected] within 1 week from arrival.

  1. J-1 Check-in Forms - completed and signed
  2. I-94 record printout
  3. Copies of J-1 visa page and Form DS-2019 used to apply for the visa
  4. Proof of valid medical insurance showing your name (and all J-2 dependents) and validity.  (Postdoctoral fellows who are employees at City of Hope do not need to submit proof of valid medical insurance)

New F-1 Check-in Procedures: Once you arrive at City of Hope, please send the following documents to Immigration Services at [email protected] within one week from arrival.

  1. F-1 Check-in Forms completed and signed
  2. I-94 record printout
  3. Copies of J-1 visa page and Form I-20 used to apply for the visa

New H-1B Check-in procedures:  For H-1B holder arriving from outside the United States, please send the following documents to Immigration Services at [email protected] within one week from arrival.

  1. I-94 record printout
  2. Copy of H-1B visa page

There are no check-in procedures for H-1B holder changing status or changing employer in the United States.

New E-3 and TN Check-in procedures: For E-3 and TN holders arriving from outside the United States, please send the following documents to Immigration Services at [email protected] within one week from arrival.

  1. I-94 record printout
  2. Copy of visa page

Other visa holders: There are no check-in procedures for individuals holding other type of visas.

Apply for Social Security Number

Individuals who will earn incomes from City of Hope need to apply for a social security number.  You can only apply when you are physically in the United States and after you complete the check-in procedures above with City of Hope’s Immigration Services.  You do not need a social security number if you will not be earning any income from City of Hope.

Note that the Social Security Card will be mailed to you, so please be sure to provide a permanent mailing address on the application form.
