Application Now Open

A complete application consists of Part 1 and Part 2.  Part 2 includes waivers, and all the supporting documents.
 
Once Part 1 is completed, the system will send you a link to complete Part 2.  When both parts are completed, the applicant must Click Submit to complete the enrollment. The application is multiple pages long and can be completed in one sitting or on multiple attempts.  If you are not able to complete the application in one sitting, the system will provide a RETURN CODE.  We recommend the applicant write the Return Code down and save it.
 
When coming back to the application, use the link above, then look at top right corner for Return Code Link.  You will then be directed the last saved question.
 
Only Completed Application Packets will be considered for review by the Intensive Course Application Committee. 
 

Required Supporting Documents

The following are the required supporting documents.  To find samples of a professional biography and photo, you must open/start application link.  The samples are available at the end of the application.
 
The application also has the Letter of Support format and instructions.
  • Letter from your institution indicating that protected time will be provided for course participation, and statement that you will have institutional support to provide cancer risk assessment upon return to your community (if applicable)
  • Current C.V.
  • One-paragraph professional narrative
  • Current headshot
     

Acceptance

Emails notifying applicants about acceptance decisions will be sent as early as the end of August and as late as the end of September. The nonrefundable course fees are due on or before the end of October.
