EpicCare Link

EpicCare Link is a secure, HIPAA-compliant, web-based tool that connects external physicians and clinical teams to City of Hope. Users can access EpicCare Link anywhere with an internet connection and computer. For technical support, call 626-218-4357 (626-218-HELP).

Who Can use EpicCare Link?

EpicCare Link is for:
  • City of Hope referring physicians and associated office staff
  • Community-based care partners, including hospitals, medical groups/IPAs, private practices and transitional/long-term care facilities

What are the benefits of EpicCare Link?

You can use EpicCare Link to:

  • Attach electronic records in real time
  • Proactively track patient referrals, appointments and more
  • Have secure two-way communication with City of Hope physicians and link your message to patient documents or charts for ease of reference
  • Maintain continuity of care with patients at City of Hope
  • Coming soon: Request electronic patient referrals to any City of Hope location, as well as submit patient documents

Simplified Referrals and Patient Status Tracking

  • View progress notes, medication lists, discharge summaries, orders and test results on demand without needing to request records to be faxed or mailed.
  • Track status of submitted requests.
  • Reduce care duplication.
  • Coming soon: Create referrals to any City of Hope location and track past and future patient appointments.

How do I enroll in EpicCare Link?

In order to register for EpicCare Link, you'll need:

  • NPI number (clinical staff only)
  • License number and state license issued (clinical staff only)
  • Site name, address, phone and fax number
  • Your first name and last name
  • Your work email
Enroll in EpicCare Link now. Once approved, we'll email you a confirmation and login information.
