Step-by-step instruction
City of Hope makes it easier to Check-In for your appointments. When you arrive for your appointment, you can self-check-in by following the step by step instructions on the screen.
For patients visiting the City of Hope Orange County Lennar Foundation Cancer Center only
Start the Check-In
1. Click Start to begin.
- Select Español to change the Welcome language to Spanish.

2. The next step is to choose if you are the patient, or “Someone else,” a family caregiver or a Proxy checking the patient in.

Checking Your Identity
1. Touch the pencil-paper icon to open the keyboard to type.
- Touch Or use a barcode to log in to log in with a barcode. See below for step by step instructions on how to log in with a barcode.
2. Enter the patient’s first name and touch OK.
3. Enter the patients' last name and touch OK.



4. Enter the last four digits of the patient’s phone number to confirm patient’s identity.
5. Enter patient’s date of birth.

Check-In Using a Barcode
1. If you completed PreCheck-In on MyCityofHope before your appointment, you will have received a barcode (also known as QR code) that you can scan on the tablet to start your check-in.


2. Hold the barcode to the tablet to start your check-in.

MyCityofHope Sign Up
Once signed in, by barcode scanning or typing information into the tablet, you will continue the check-in process. If you do not already have a MyCityofHope account, you can sign up now by:
- Print activation code
- Text message (SMS) instant activation
- Email activation

Patient Health Summary
Here, you will be asked to fill out a questionnaire that will take a few screens. Click the arrows in the middle of the screen to move to the next page.

Notice of Privacy Practices
Next, you will be asked to review and Accept the Notice of Privacy Practices

eSignature (Electronic Signature)
Sign your name in the box. Touch OK to accept the signature. Touch Clear to erase the signature and try again. Touch Cancel to close the signature window.

If someone other than the patient signs this document, select their role below the signature.
After you have electronically signed the consent form, it will ask you if you want to print or email the document.


Demographic Self-Report (additional personal information)

You Can Now Easily Get Your COVID-19 Vaccination Credentials In MyCityofHope! Web+Mobile App

As people start to go back to work, travel, or attend events, you may be asked to show proof of COVID-19 vaccination status.
MyCityofHope now gives you access to your verified COVID-19 vaccination and test result information on your MyCityofHope patient portal. You can get also print it and share it with anyone who needs it.
Introducing the new COVID-19 activity quick access button, available on the web version and the mobile MyCityofHope app.

On the web or desktop version of MyCityofHope:
- Log into your account at www.MyCityofHope.org.
- On the home page, you will see an icon with the COVID-19 image.
- Click on the icon button to easily jump to the COVID-19 activity and access your vaccination and tests.

On the MyCityofHope mobile app:
- Log in to your MyCityofHope account from the City of Hope mobile app.
- On the home page of the MyCityofHope screen touch the COVID-19 activity quick access icon to access your vaccinations and test.

Don't show me the COVID-19 activity quick access button
If you don't want this button to show on your screen, you can remove it by going to the COVID-19 activity and clearing the "Show the COVID-19 info button at the top of every page" checkbox.

You Can Now Get A QR Code To Show Your COVID-19 Vaccination Or Testing Status
California Immunizations Registry (CAIR) | California Department of Health Services
You can now get a QR code to prove your COVID-19 vaccine and testing information through MyCityofHope!
The QR code contains your legal name, birth date and either your COVID-19 vaccination as recorded in your medical record or the most recent COVID-19 test result.
You can access the QR codes from a mobile device, export information from the QR codes to another app or print out, QR codes from MyCityofHope.org.
You can see your vaccine and testing information, including the QR code on the MyCityofHope mobile app without having to log in to the mobile app. Touch “Vaccination Complete” to see your information.

Not seeing your vaccine?
If you don’t see your COVID-19 vaccination information in MyCityofHope, you can submit a request (or query) to the California Immunization Registry (CAIR) to pull your COVID-19 vaccination information directly into MyCityofHope.
If some of your vaccine information is missing or incomplete from CAIR, you can send a message to your care team to confirm your complete vaccination status. Note: Your vaccination will not be confirmed until your care team verifies it.

COVID-19 Vaccine Questionnaire
If your request (or query) to CAIR doesn’t return any results or if you the discard vaccination information received from CAIR, you can simply fill out the COVID-19 Vaccination Information questionnaire to submit your vaccination information.

Complete the Questionnaire and Upload an Image of Your Vaccine Card
You must answer the questions in the questionnaire and upload a picture of your vaccination card as proof of vaccination.

You Can Now Prepare For Upcoming Tests And Procedures
MyCityofHope now gives you a page with a new activity that shows all your upcoming test and procedure orders in one place.
We have also added a new card to the health feed to help you keep track of upcoming orders and easily get to the Upcoming Tests and Procedures activity when you need to follow up.

If you are not ready to schedule the test yet, you can "snooze" the reminder to hide the card from your health feed for up to 90 days.

The New Message Center
Finding old messages or sending new ones is now easier than ever in the MyCityofHope Message Center.
In the Message Center, you can:
- View conversation threads in order — You can now easily find a message you need, because conversations with the same subject and email recipients are bundled into conversation threads.
- Bookmark conversations for finding them faster in the future — If you know that you want to find a specific message or conversation thread easily in the future, you can bookmark these conversations for easy future access.
- Search for messages by keyword.
- Quickly find questionnaires or schedule tasks you need to complete.
- Add attachments in messages to your care team.
Requests for medication refills are now in Medications (no longer in messages).


To send a new message to your health care team from the Message Center.
- Select the message topic and recipient.


2. A window opens where you can type your message and attach documents or images.

MyCityofHope gives you folders to keep your messages organized.
These are:
- Conversations —This is the main message folder where new replies and nonautomated messages arrive.
- Read receipts — Read receipts are available now and each message in the conversation will be marked read as it happens.

- Bookmarked — This folder includes messages that have been Bookmarked by you to revisit later. For example, a message from a physical therapist with ongoing exercises to complete may be a good one to bookmark.
- Appointment — This folder has appointment-related messages, such as appointment scheduling requests or cancellations.
- Automated messages— This folder includes messages sent automatically, such as payment confirmation or questionnaire submission confirmation messages.
- Trash— This folder includes messages that you have deleted. These messages can be restored to the main conversation list if needed. You can't permanently delete messages in this folder without assistance from your organization.
More New Things You Can Do In The New Message Center!
Happy Together — In each of the folders, you can see messages from other hospitals or health care providers using a Happy Together connection.
Message Details — If you click on one of these messages, you will be taken to a Message Details page that shows the message came from an outside hospital or health care provider.
Search by keyword — If you have a lot of messages in one folder, you can search by keyword to easily find a specific message. The search can find matches based on the message subject, body text, or author
Action Needed — Any message that has a task that you have to do, like respond to a questionnaire or schedule an appointment, shows the task from the conversation list.
This way, you can quickly see when you have an action to take without having to click on the message, scroll, or search. After you have clicked the message, you can see who you are messaging in the Participants window. If anyone else, like a proxy, can see the conversation, that person's name will show in a Visible To section of the window.
If a proxy has access to your account and you allow the proxy to see the message you are sending, the proxy will receive a message notification when you send the message. This helps the proxy stay informed about your care. One more way, MyCityofHope helps your health care team, including you and your family caregivers stay connected.

PreCheck-In [eCheck-In]
MyCityofHope saves you time with PreCheck-In for appointments from the comfort of your home. Taking a few minutes to complete the PreCheck-In process lets you and your health care team focus on the important things during your visit. You can complete PreCheck-In anytime between one week to 24 hours before your appointment.
With PreCheck-In, you can:
- Verify or update home address, email address and phone numbers.
- Verify and update medications, allergies and current health issues.
- Verify and update insurance information and upload your insurance card image.
- Pay your insurance copay with a credit or debit card.
- Electronically sign required forms or documents.
- Complete previsit health questionnaires.
Steps for PreCheck-In:
- “Sign in” to your account on MyCityofHope.org.
- Visit www.MyCityofHope.org
- Enter your MyCityofHope user name and password. Click “Sign in.”
- Click “Visits.”
- Click "My Upcoming Appointments."
- Pay your insurance copay with a credit or debit card.
- Choose your appointment and click on “PreCheck-In” or “Update Info” to start.
Start PreCheck-In
Click PreCheck-In to check in to the appointment listed on the screen.

Personal Information
Now, let’s verify your personal information. Are your address, telephone number and email address correct? If you need to update your information, click "Edit" and enter the new information. If correct, click on "This information is correct" to check the box and click "Next."

Travel
Did you travel since your last appointment? If yes, please add a trip. If not, leave blank and click "This information is correct" to check the box and click Next. To Add a Trip, click on the drop down menu to select country.

Enter the date of your trip. Click "Accept."

Click on "This information is correct" to check the box and click "Next."

Insurance
Next, verify that you that your insurance is correct.
Click "+Add a coverage" to add new insurance or change your current insurance on file.
Click "This information is correct" to check the box and click Next.

Payments
Do you have co-pays for this visit?
You may pay your co-pay during PreCheck-In or choose to "Pay copay later" and click "Next."

Medications
Let’s review your medications. Are all of your medications listed? Now, select a pharmacy and confirm the information is correct.
Click on "This information is correct" to check the box and click "Next."

Allergies
Please review to make sure all of your allergies are listed. Check that the information is correct.
Click on "This information is correct" to check the box and click "Next."

Health Issues
Finally, let’s review your health issues on file. Check to see if everything is up to date.
Click "+Report a health issue" if you would like to add a new health condition.
Click on "This information is correct" to check the box and click "Next."

Now, let’s take a few minutes to report current symptoms you may be having.
Communicable (Infectious) Disease Screening

Click Continue.
Answer the remaining questions about symptoms and click Submit

Please answer the questions on this page to the best of your ability. Click "Continue."
Continue to answer the questions about current symptoms. Click "Continue" and "submit" when completed.


Patient Health Summary
Next, please check your health summary history. If you have had any of the listed cancer diagnosis, please click on the box to select them. Enter the date first noted and any comments you would like to add.
