Accessing My Health Information/Medical Records

When can I see my test results in MyCityofHope?
Your test results will vary when they are released to your MyCityofHope. For example, most Outpatient labs will be available immediately after your results have been received, most Inpatient labs will be available 48 hours after we receive your results, Outpatient Radiology exams will be available 48 hours after we receive your results and Inpatient Radiology exams will be available 48 hours after you have been discharged from the hospital. NOTE: Certain test results may not be available until your provider has first discussed them with you.
 
Why are certain test results not shared electronically via MyCityofHope?
Under California law, certain test results, such as pathology, genetics, and other specialized labs, are considered sensitive and will not be shared via your patient portal. Please visit the Health Information Management Services Department website, to learn how to request a copy of your test results.
 
If some of my health information/medical records on MyCityofHope are not correct, what should I do?
Please visit the Health Information Management Services Department website and complete the form, “Request to Make a Correction to your Health Information/Medical Records”.
 
If I send a message to my doctor or nurse, when can I expect a reply?
You will generally receive an answer within one to two business days. Please note that MyCityofHope should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention, or dial 911 if it is an emergency.
 
Can I combine or see my records from other hospitals or providers in one place?
Yes, you can combine medical records from multiple health care providers in MyCityofHope, if they use MyChart technology.
 
Can I see my provider’s notes after my visit?
You can now view your After Visit Summary and the clinic notes that your health care provider made after your visit. You can also view any of your doctor’s visit notes that are shared with you by selecting the View Notes in MyCityofHope.
 
What if I have a FollowMyHealth account?
As of Dec 2, 2017, City of Hope no longer updates your health records or supports FollowMyHealth. If you would like to access City of Hope health records prior to December 2, 2017, please call the MyCityofHope support line at 844-777-4673 or email mycityofhope@coh.org.
Technical support is available 24 hours per day/7 days a week.
 
You can still access your FollowMyHeatlh account for other hospitals and providers. If you have any questions about medical records from other hospitals or providers please contact FollowMyHealth or the specific hospital or provider directly.
