Upon form submission, you will receive an automated confirmation from our patient intake system, including preparatory information for your journey
with City of Hope®. A dedicated member of our International Patient Care team will then contact you within two business days to guide you through the next steps, including preparing and securely submitting your translated medical records for review.
Essential Documentation for Review Process
To ensure a seamless evaluation, compile the following documents, labeled with the patient’s full name in ENGLISH and date of birth. While direct medical record translation services are not provided, City of Hope is dedicated to assisting you by connecting you with certified translators to ensure a smooth preparation process.

Tailoring Your Care
Upon receipt of all relevant medical records, our esteemed physicians and nurses will thoroughly review your file to ensure completeness, typically informing you of your status within three business days.
To achieve a comprehensive understanding of your health, additional tests or scans may be recommended before your City of Hope visit. We will also estimate your treatment cost, providing transparency on deposits and secure payment methods. Our patient access coordinators are available to address financial questions, guiding you through the financial aspects of your care.
After our clinical team completes a thorough review of all relevant medical records, a patient access coordinator will contact you with an estimated cost based on your personalized treatment plan and guide you through the secure payment process. Our team will then efficiently coordinate your consultation appointments in alignment with your treatment and travel schedule.

If you require a visa, we will support you through the process by providing a detailed list of necessary documents for your visa interview. Additionally, upon request, we will furnish a letter confirming your scheduled appointment at our facility. Our welcoming team will greet you warmly and introduce you to your personal patient access coordinator. This coordinator, proficient in your language or equipped with translation services, will ensure that all your needs are met and will guide you seamlessly to your initial appointment.
During your initial physician consultation and subsequent medical appointments, a certified translator will be available to ensure clear and effective communication.

Items to Bring for Your First Appointment
Please remember to bring the following items on the day of your first appointment:
- Medical Images: All medical images saved onto a CD or USB drive in DICOM format. If possible, please send these images in advance, and bring the CD or USB drive with you to your appointment.
- Pathology Materials: Representative samples of all pathology biopsies conducted, including corresponding blocks, as well as unstained slides and/or stained slides for clinical review.
- Biopsy Results: Initial biopsy results.
- Surgical/Metastatic Biopsy Results: If applicable, results of any surgical or metastatic biopsies performed.
Please note that certain cancer types may require additional clinical information. After you complete the new international patient form, a dedicated patient access coordinator will send you a secure link with instructions to upload your radiology images and will follow up to discuss any additional details needed.
