Hospice and Palliative Medicine Fellowship Program Application Process

Our retention policy is based on the ability to meet the clinical and academic expectations of the fellowship.

Eligibility Requirements

  • Must have completed a prerequisite residency program in a qualifying field such as family medicine, internal medicine, pediatrics, or neurology
  • Must be eligible for a California License
  • Must be in compliance with the American Board of Family Medicine ABFM or other relevant board guidelines for professionalism.

  • Register for the Match: Register with the NRMP to participate in the fellowship matching process.

Document Requirements 

  • ERAS application
  • Applicants must have completed an Accreditation Council for Graduate Medical Education (ACGME)-approved residency program.
  • Curriculum vitae
  • Personal statement describing the applicant's interest in and commitment to a career in palliative care
  • Three letters of recommendation, one of which should be from the applicant's department head, program director, or division chief
  • USMLE (U.S. Medical Licensing Examination) transcripts
  • Headshot Photo
  • Graduates from a medical school outside of the United States or Canada must submit a valid ECGME certificate
