MyCityofHope FAQs

A portal is a secure and private online tool that gives you access to your personal health information.

As a patient, you control information in your portal and who has access to your health information.
 
For that reason, this portal is called MyCityofHope.
 

Once you are registered, you may want to consider either bookmarking or saving MyCityofHope to your favorites.

Enrolling is fast and easy! All you need is an email address to begin.
 
Email Invitation: To activate your MyCityofHope account, request your email invitation today at registration or in the Sheri & Les Biller Patient and Family Resource Center. If you have an upcoming appointment and have an email address on record with City of Hope, you will receive an invitation email to activate your MyCityofHope account. You will be required to enter an invitation code that will be included in the invitation email. Once you receive the invitation, simply click the link to the portal and follow the three steps to register.
 
It is recommended that you complete your initial registration on a laptop or desktop computer as this process requires significant system resources and time to compile your clinical information into the portal. Once you are registered, you will be able to easily access your information from your smartphone, tablet or computer.
 
In Person:
  • Visit the Sheri & Les Biller Patient and Family Resource Center for personal assistance in registering your account. Ask any staff member for directions to the nearest registration location.
  • If you are visiting a community practice site, speak with the receptionist for assistance.
 

Any patient of City of Hope can enroll by responding to the invitation email and following the instructions on how to register.
 
If you are a proxy for a minor child you will have access to the minor’s medical record until the minor turns 18 years of age. After that point, you will only have access to historical information and will be unable to view any new health information. Both parents are allowed to have access to the same child’s account.
 
Once the minor turns 18, he or she will be able to create an account to have access to his or her health information.
 

It depends.  MyCityofHope is built on the FollowMyHealth™ Universal Health Record product. If your other portal(s) also uses FollowMyHealth, then yes, you can simply add City of Hope to your portal and have access to both. If not, you would have two separate portals.
 
If your existing portal is also a FollowMyHealth portal, when you enter your email address to register a message will display if that email is linked to an existing account. If so, log in to your existing portal account and add City of Hope as a provider to be linked to your City of Hope health information.
 
Also, please note your demographics, such as name, address, insurance and preferred pharmacy, is downloaded the first time a FollowMyHealth account is created. If you have previously enrolled with another health institution that uses FollowMyHealth, the demographic information will reflect the information provided at that time.

Once you register, your health information from all City of Hope physicians you are actively seeing will be available for viewing.

Yes. All data is encrypted to ensure privacy and security. The portal meets the industry standards for secure Internet communications

At this time, the language setting can be changed to Spanish. When the setting is changed the headings and non-medical text will display in Spanish, however, the clinical information will be displayed in English.

Registering for and using MyCityofHope is complementary and is offered as a service to our patients.
