Health Insurance Plans Los Angeles-Area Locations Accept
Health care insurance may feel complicated. This page answers some common questions about insurance coverage.
Each patient’s medical situation and insurance plan is different. Although there is no guarantee of coverage, you should ask your health insurance company to review your case with City of Hope®. Even if your plan doesn’t cover care at City of Hope Cancer Center Duarte and surrounding locations, you can ask your insurance company to authorize treatment here.

The following is a list of insurance companies that contract with City of Hope National Medical Center (hospital and technical services at the Duarte cancer center only) and City of Hope Medical Foundation (physician services only at the Duarte cancer center or another Los Angeles-area clinical location). If you don't see your specific plan or product below, please contact the company customer service department to check if you're covered or that City of Hope Medical Center or City of Hope Medical Foundation is a participating provider.
A PPO contracts with doctors and hospitals to create a network of participating health care providers. You save money when you use providers in your PPO network. If you use out-of-network providers, you may incur additional costs.

Your coverage only extends to doctors and hospitals within your EPO network. There are no out-of-network benefits.
This health plan option allows you to choose a service from a participating or nonparticipating provider, with different benefit levels associated with each.
If you belong to an HMO, your insurance company will not pay for your health care unless you have an approved authorization from your insurance company and/or medical group prior to a planned admission or any service.

Medicare is a federal health insurance program for people aged 65 years or older or with disabilities. It covers the costs of hospitalization, medical care and qualifying related services at City of Hope.
This plan allows Original Medicare beneficiaries to receive covered benefits through HMOs and Preferred Provider Organizations (PPOs) instead. Private insurance companies offer additional benefits, such as dental, prescription drug and vision coverage, gym memberships, etc. The HMO plan requires the beneficiary to choose a primary care physician.

Dual Eligible/Cal MediConnect/Medi-Medi is a voluntary program for people with both Medicare and Medi-Cal. It benefits seniors and people with disabilities and will combine medical, behavioral, long-term and home- and community-based services in one health plan. If you belong to an HMO, your insurance company will not pay for your health care unless you have an approved authorization from your insurance company and/or medical group prior to a planned admission or any service.

Medi-Cal is a public program that provides health care coverage to adults, families and people with disabilities who meet the income requirements. If you belong to an HMO, your insurance company will not pay for your health care unless you have an approved authorization from your insurance company and/or medical group prior to a planned admission or any service.

If you receive care from a provider without a contract with your insurance company, that service is considered out of network. You may be financially responsible for any additional costs.
A provider-based facility is a hospital outpatient department of City of Hope National Medical Center, a general acute care hospital in Duarte, California, and a National Cancer Institute-designated comprehensive cancer center. Patients benefit because we offer additional resources for them and their families, including clinical nutrition, social work, rehabilitation/physical therapy, psychosocial screening and other related services.
