Financial Assistance at City of Hope
Patients and their loved ones sometimes incur substantial financial needs and cannot afford care. They may be eligible for assistance under City of Hope's Financial Assistance Policy and Patient Discounts and Free Services Policy.

How to Apply for Financial Assistance at City of Hope
To be eligible for financial assistance at City of Hope, we consider a patient's situation, including but not limited to:
- A primary diagnosis of cancer, diabetes, HIV/AIDS or hematologic disease or treatment with hematopoietic cell transplantation
- Income and assets
- Ability to pay self-pay balances
Patients must submit an evaluation form (available in English | Spanish | Chinese) to initiate the application process before, during or after treatment. They can do this via the patient portal, by mail or visiting in person at City of Hope, 1500 E. Duarte Road, Duarte, California. Additionally, they must provide various documents to support their financial circumstances and proof of income. Paper copies and translations of our policies, plain language summary and evaluation form are available upon request and free of charge.
Applicants receive a written decision about their eligibility and, if denied, may appeal decisions. If circumstances change, patients may resubmit an application. Applicants may also request prescreening and application assistance from our financial counselors.
How to Apply for Patient Discount
There are no specific eligibility requirements for patient discounts. Discounts will be applied to your bill if you meet specific coverage criteria.
- If you opt to self-pay (e.g., without insurance), a discount will automatically be applied to your rendered service(s) without needing an application.
- If you are covered with insurance but require noncovered services, a discount will also be automatically applied to those services without needing an application.

Financial assistance and discount payment information are available in other languages. City of Hope is committed to using the primary languages of its major ethnic and cultural communities in all materials connected with the Financial Assistance Policy and the Patient Discounts and Free Services Policy. Printed information is available in English, Spanish, traditional Chinese and more. Translated versions of the relevant materials are available upon request.

Billing and Other Financial Resources at City of Hope
You are protected from balance billing when you're treated by an out-of-network provider at an in-network facility. In these cases, you shouldn't be charged more than your plan's copayments, coinsurance or deductible.
We've made our pricing available to patients because we believe in creating transparency and providing access to crucial information, such as City of Hope's shoppable services and standard charges, as they make important decisions about their care.

Many legal issues may arise during cancer treatment, including health insurance options, employment rights and reasonable accommodations in the workplace, taking time off work, and access to disability insurance and government benefits.
Financial Assistance Resources Beyond City of Hope
The Hospital Bill Complaint Program is a state program that reviews hospital decisions about whether you qualify for help paying your hospital bill. If you believe you were wrongly denied financial assistance, you may file a complaint with the State of California’s Hospital Bill Complaint Program.
