Is My Insurance Accepted?
View a list of insurance companies that City of Hope currently contracts with for its locations and physicians. Even if your plan is included, check with your insurance company to make sure you are covered. Your insurance company/medical group will tell you if you need any authorizations.
If you wish to receive treatment at City of Hope, but we're not currently contracted with your insurance plan, contact your insurance company and ask about obtaining authorization to receive treatment here.
No Surprise Billing

Bahareh Bahadini, M.D., is an assistant clinical professor at City of Hope's Thousand Oaks location.
Amy Hakim, M.D., is a surgical oncologist dedicated to treating gynecological cancers. In addition to providing gynecological surgery options to treat cancer, Dr. Hakim is also studying the connection these cancers’ connection to then human papillomavirus (HPV) and the HPV vaccine.
Ashwin Kashyap, M.D., is a medical oncologist at the City of Hope Thousand Oaks location.
David Litvak, M.D., M.B.A., is an assistant clinical professor, Department of Surgery.
Nikita Shah, M.D., is an assistant clinical professor in the Division of Breast Surgery, Department of Surgery.
