Aging Wellness Consult Clinic

This team of advanced practice nurses specializes in caring for older adults with cancer. The clinic visit includes a complete assessment and education for the older adult based on the guidelines developed by the American Society of Clinical Oncology. These guidelines have been shown to prevent or minimize problems during your cancer treatment.

Our clinic team will work with your primary oncology provider, along with Physical and/or Occupational Therapy, Nutrition, Pharmacy, Clinical Social Work and Supportive Medicine to improve your care.

Age-Friendly Health Systems

All older adults deserve safe, high-quality health care that is based on what matters most to them as individuals. City of Hope’s OASIS Program is leading the way in making sure that every day, every older adult receives age-friendly health care. As part of the Age-Friendly Health Systems movement, we are among the first cancer centers in the country implementing age-friendly health care.

What To Expect

We will ask you to complete a health survey online before you come to the clinic, so we have this information to plan your visit. This survey will give your team information about your overall health, physical function and activity, social support, nutrition, medications and health conditions that are common in older adults. When you are at the clinic, a team member will help guide you to perform several tests—usually a short walk and thinking test. We may ask you to do more, if needed.

Clinical Highlights: Holistic Approach To Wellness

During a visit, either in clinic or via telehealth, the advance practice nurse will review:

  • Functional status (ability to do physical activities)
  • Comorbidities (other noncancer conditions, such as diabetes, heart diseases, etc.)
  • Cognitive function (memory concerns)
  • Nutritional status (nutrition/diet concerns)
  • Psychological state (emotional well-being concerns)
  • Social activity/support concerns
  • Polypharmacy (review of medications, vitamins and herbal supplements)
  • Advanced care planning (goals of care, advance directives)
